Make an appointment by calling us or sending us a message here. While booking the appointment you will be asked your credit card information for a $150 appointment deposit. You will also be given instructions on the requirements for the day of the appointment, including form I-693.

You can download the form here. Fill out the form following the instructions that were sent to you by the Foreviva team, and add it to the required documents for the day of the appointment.
Come to the appointment
Arrive on the day at the appointment at least 15 minutes earlier, and make sure that you bring with you all the required documents needed for the exams.

How much is the deposit?

What if I already have some of those tests or exams completed?
- If you have valid vaccine records or certificates that prove the completion of those exams or vaccines, then you don’t need to receive them again.
What if I don’t remember if I have some of the vaccines?
- On the day of your consultation, we will perform an blood test that indicates which vaccines you have and which are missing.
